An empty 5.56mm bullet cartridge, consistent with ammunition used in SAR-21 rifles, was discovered along Yishun Street 81 outside Orchid Park Secondary School on the night of September 22, 2018. A passerby alerted police at 8:09pm after finding the round near the school. The Straits Times and STOMP reported that witnesses in the area claimed to have heard "a super loud explode sound" around the time of discovery, though the cartridge itself was empty.

Police cordoned off the area and launched an investigation. Local singer-songwriter Kohji Toh documented the scene with videos showing police lights and vehicles stationed outside the school.
